Have an outpatient therapy appointment?

Prior to your initial visit, you can minimize your wait time by completing our intake forms before your appointment.  Print out the forms that pertain to you and bring with you on your first visit. 

Patient Information Form (Adult)- If you are age 18 or over, please complete this form.

Pediatric Patient Information Form- If you are age 1-17, please complete this form.

Authorization to Treat- All patients should complete this form.

Prior Services Questionnaire- If you have Medicare, Medicaid, or Carolina Access as your primary insurance, please complete this form.

Pelvic Pain Form- If you have a pelvic pain therapy appointment, fill out this form.  (You do not need to fill out the Patient Information Form)
